There was no close inspection of the "ground" connection on this video.
But, there is another piece of evidence, that is against the hot/live wire inside the braid (or Litz'ed braid).
Namely, that clamp-on meter would read zero Amps, if both the hot wire and return wire were inside the meter's jaws.

That leaves the possibility of a hot wire being surreptitiously connected on the load side somewhere.  The video does not reveal that...
...or the device is really working as advertised.

The hidden live wire inside the ground wire theory doesn't seem so likely to me given that people not connected to Kapanadze
have inspected the ground connection and have seen nothing suspicious there. As I said however, it doesn't mean it is not a fake,
but it would appear that the ground wire connection is not so likely where any tricks are being used. Could be a trick with the
output wires/load wires, but presumably people have taken a close look at those as well when they visited in person.
Kapanadze seems not really so easy to dismiss, to me anyway...
