DC can be created from an AC supply. I am sure that is why the AC references are there. Ostensibly, his devices require a DC input. In the simplest case, a battery or charged capacitor (accumulator) provides this input.

Some of my thoughts (you may confirm or reject them)
1) without the hv winding it would be a normal stepup transformer
(12 V to 220 V)

2) The HV AC frequency is coupled via PLL to the low frequency : 50 hz
so that the HV AC is a integer multiple of this main frequency.

Now there are 2 things to experiment with:
a ) Main Voltage (50 Hz) should be high enough to create a strong magnetic field .
Saturating the core leads in my opinion only to thermal losses.
b ) varying the multiplier and notify if the output increases.
